The PHO Performance Programme has been developed by District Health Boards (DHBs), the Ministry of Health and the primary health care sector to support improvements in the health of people enrolled in a Primary Health Organisation (PHO).
The Programme aims to:
DHBs contract PHOs to deliver a range of health care services for people when they are unwell, to help people stay healthy and to reach out to groups of people in the community who have poor health or are missing out on primary health care.
The Programme has developed a number of performance indicators to measure PHO achievements over a six month period. These PHO achievements have been grouped together by DHB region so that the Programme is able to report on services accessed by PHO enrolled populations within each DHB region.
Some performance indicators measured by the Programme look at services accessed by all PHO-enrolled patients within a DHB region while other indicators look at services specifically accessed by Maori or Pacific Island people or those living in lower socio-economic areas. These patients are referred to as ‘high need’ patients. Evidence has shown that ‘high need’ patients have poorer health than non-Maori or non-Pacific Island people or people who do not live in a lower socio-economic area. One of the Programme’s main objectives is to reduce the health ‘gaps’ between high need and non-high need patients so that all New Zealanders, whatever their ethnicity or living standard, can access the health services they need in order to be healthy.
The links below the map provide the PHO Performance Programme performance achievements for each of the 20 DHBs in New Zealand plus the 39 PHOs plus South Canterbury Primary & Community Services reporting sepately during the reporting period. The DHB and PHO achievements are calculated by matching health information (some pertaining specifically to patients) sourced from PHOs and the Ministry of Health. The Programme has maintained privacy requirements when accessing, matching and reporting the data. The DHB-level results are calculated by taking into account the performance of all PHOs within the DHB’s area of responsibility and comparing those results to the overall national performance results (i.e. the combined results of all 60 PHOs plus South Canterbury Primary & Community Services).
